Patient Resources

Prior to admittance, we will submit and verify the patient’s current insurance information. If there are any difficulties with insurance approval or authorizations, we will notify patients prior to any medical procedures. For questions or concerns regarding pre-authorization please call 920.262.4210.

The following information is needed when scheduling as a patient at Watertown Regional Medical Center. To schedule please call 920.262.4298.

• Physician order
• Insurance information
• Please expect to allow 10-15 minutes for scheduling call process

The following documentation is required when registering as a patient at Watertown Regional Medical Center.

• Current insurance card(s)
• Photo I.D.
• Social security card
• Advanced directive/Living Will (if applicable)

A patient, parents of minor patients, or caregivers acting on the behalf of a patient (i.e.Healthcare Power of Attorney), may inspect and/or obtain a copy of their medical records, or have copies of medical records sent to another provider or facility.
