A patient, parents of minor patients, or caregivers acting on the behalf of a patient (i.e.Healthcare Power of Attorney), may inspect and/or obtain a copy of their medical records, or have copies of medical records sent to another provider or facility.

Obtain Printed Copies of  Your Medical Records

For questions regarding your medical records, please call 920.262.4370. Patients looking to obtain a hard copy of their medical records can download the “Release of Information” form below and fax the completed form to 920.390.7959 or email it to roi@watertownregional.com.

  • Photo Identification is required.
  • Patients must completely fill out and sign the release of information authorization form.
  • A fee will be charged if requesting a copy for personal records. If records are being sent directly to another provider or healthcare facility for continued care, the fee will be waived.
